Order A Replacement Membership Card

There is a $15 fee for a replacement of a hard copy USEF Membership card.  An online card is free. Click on Print Proof of Membership for Competition Purposes for instructions to print the online membership card.

In order to request a replacement card on line:
1. Log in to your My USEF Account.
2. On the Member Forms menu, select Order 2009 Replacement Membership Card.
3. You will see the payment summary indicating that you will be charged the $15 fee.
4. Enter Billing information as requested.
5. Click Continue. If the transaction was successful, you will receive a message stating that your transaction was successful and you will receive your new card shortly. If the transaction was not successful, you will be notified and asked to re-enter a credit card number and billing information.
