Horse Recording / Horse ID Application
You can request a horse recording or horse ID using the on line application.

Horse Recording
In order to record your horse on line:
- Log in to your My USEF Account.
- On the Member Forms menu, select Horse Recording Application/ID.
- Read the notice. Click Continue.

In Competition Renewal: If you complete an USEF online horse recording application
in the competition office, on your lap top in the barn or hotel room, or home computer during
a competition, you must put the competition number in this section of the online application
in order for your membership to have the start date of the competition and Farnam®/Platform™ •
USEF Horse of the Year (HOTY) points to count. Ask the competition secretary for this number.
If you are not at a competition, hit continue.

Under each Category (Horse ID, Life or Annual), select in the drop down box what action
is needed. Example: Annual Recording ($75). For any horse where no action needs to be taken,
simply select "no change." At the bottom of the screen type in your name as requested.
Click on Add New Horse.

On the next screen please type in your name verifying that you are the primary owner
of the new horse. Click Continue.

If you are trying to transfer ownership, record a lease or complete a name change of a
horse with an active recording, please click on the link in the top box that states "Click
here to download applicable form to
report these changes as you cannot complete these transactions on this online procedure.
To continue with the online horse application: Complete the application form. Required fields
are marked with an asterisk (*). If the horse was previously recorded or has a USEF ID, enter
the number in the USEF Horse's ID field.. Click Applicable box at bottom of page. (Add Another
Horse, Add Horse and go to Payment Screen, or Do Not Add Horse and Continue)

When finished and you are at the Payment Summary Screen: Your horse(s) will appear in the
box at top of page. Your total charge will appear under Payment Summary. Under Payment
Information, enter your credit card type (USEF accepts VISA and MasterCard), expiration date,
and card number (16 digits with no spaces or other characters). Also input the zip code for
the billing address of the credit card and credit card holder's first and last name. The
card code is also required. This is the 3 digit number found on the back of your credit
card.

Click ONCE to Continue. If the transaction was not completed successfully, the program
will not go to the receipt step (next step). You can enter another credit card number and
try again. If your transaction was completed successfully, you will go to the receipt page.

If your transaction was completed successsfully, print the receipt page for your records.
Click the link for an online Horse Recording card or a Horse Recording 45 day Show Verification.
Horse ID
In order to request a horse ID on line:
- Log in to your My USEF Account.
- On the Member Forms menu, select Horse Recording Application/ID.
- Read the notice. Click Continue.
- Read the information at the top. In Category, select Horse ID (No charge).
- Complete the application form. Required fields are marked with an asterisk (*).
- Click Continue.

Read the Notice (clicking Continue indicates the acceptance
of the terms in the Notice). The payment summary will display
a total of $0, indicating no charge for the horse ID. Click
ONCE to Continue.
- You will get a response indicating no charge. Click Continue.

You will be notified that your horse ID information was
submitted. You can click the links to either record or ID
another horse or return to the My USEF Account menu.
